I know of no mental or physical problems that may affect my child’s ability to safely participate in this camp. I hereby waive and release the Patriot Wrestling Camp and Wood County BOE from and against any and all claims, injuries, demands, actions, or cause of actions arising out of the participation by the camper in the Patriot Wrestling Camp. I will be responsible for any medical and other charges in connection with my child’s attendance at the Patriot Wrestling Camp. I have read the rules and regulations of the camp and both the camper and I agree to abide by them.
